ParticipantI have a new pt, 37 yo Male, with metastasized Adenoid Cystic Carcinoma. He was told a year and a half ago that there are no decent chemo or radiation options for him, so not doing any western treatments. It was found 5 years ago when he was wheezing and a tumor in his lung was discovered. That lung was subsequently removed. Now he has a 6.5cm tumor in his liver and 8-10 small lesions in his peritoneal cavity. CC: Pain in lower abdomen that feels sharp and burning (almost near zi gong xue and close to inguinal area), and duller pain in lower back R side (posterior to liver), and pain in R side neck. Scans do not show any tumors in neck/shoulder. Pain fluctuates, feels deep, better with warmth and massage. Feels worst at night, with hunger, and with cold. Other S/Sx: fatigue, insomnia d/t pain and mind runs at night, craving animal proteins (currently vegetarian), weight loss (hard to tell if d/t diet change or cancer), stress, eczema that comes and goes but starts red and is dry and itchy. Nightsweats for a few days last month, not regularly. BMs and urination normal. Pulse: L1 mid moderate, L2 wiry thin slippery upper level, L3 mid wiry. HT/KI not comm. Rapid overall. R1 deep with roll-up, R2 big hollow, R3 wiry deep. Abdominal palpation reveals no temp changes at surface, hard nodules in lower abd, rikyu. I think the pulse is telling me to start with shaoyang: XCHT – HQ + GZFLW + GZGCLGMLT, with ML at 30 and LG at 60 perhaps? Too watered down though? I know we treat tumors as abscesses, and we need to melt them and warm them. But I don’t feel an abscess pulse yet in that L Cun. For progression, I can see a TXS + GZFLW pulse there, but is that strong enough in this case? I suspect I should focus more on the material aspect rather than functional in his case.Thank you!

GuestFirst to harmonize from that pulse and the hypochondriac pain sounds good to go with XCHT, but why take out the HQ? Does he have afternoon fever? You may start to go stronger on blood movement right away by adding TBC and SZ to try and eat away the masses, but I have no experience with that kind of cancer.

ParticipantHe needs to take bulk herbs, especially at this stage. Cannot do much without it.

ParticipantYes I figured. Am I on the right track with first formula, and should I just keep GZFLW as the blood mover in there or add TBC, SZ?
